MSAA would like to learn more about the MS community that we serve. This will help us better tailor our programs and services to meet the various needs across all demographics of individuals affected by MS.

Please note: all of the information shared in this survey will be confidential. MSAA relies on grantmaking to support our programs and services. Your help in completing this survey will assist MSAA in our grantmaking activities and further our mission as a leading resource for the entire MS community, improving lives today through vital services and support. If you are responding to the “Please Tell Us More About Yourself” ad on the back of the Winter/Spring 2022 issue of The Motivator magazine that you received from MSAA in the mail:

1. Please have the issue in front of you as you complete your survey responses.

2. You will be asked to fill in a required “CON ID” number, which is unique to your specific contact information with MSAA. This can be found on the back of your copy of the magazine alongside your mailing address. You must enter ‘CON-‘ and then that unique number when asked at the beginning of the survey. For example:
